He was driving his Range Rover home from dropping his children off at school when he collided with a van. The accident occurred in High Legh (Chelteseshire). Samuel was killed instantly, and the driver of the other car sustained serious injuries. Aston Villa paid tribute to Samuel by wearing black armbands against Middlesbrough in their Championship play-off match that evening. Egerton FC cancelled all of their remaining games that week.
Inquest into Samuel’s death was held in November 2019. It revealed that Samuel had been twice the legal limit for drink driving and that his car had crossed on the wrong side. This led to a head-on collision. Samuel’s body had been so badly burned in the accident, that it was necessary to identify him using dental records and a sample of blood that matches the DNA from his hairbrush. Samuel’s sister accused his widow, claiming that she had faffed his death. She demanded that her sister receive the samples for testing.
